9/30/2023 EnchantaVaughn Approved QMHPA arrived at the home to conduct individual session with the Ms. Brown. QMHPA engaged Ms. Brown in a discussion to determine how her afternoon was. Ms. Brown verbalized to QMHPA her afternoon was going well. QMHPA informed Ms. Brown the topic of this afternoon session. Ms. Brown did not present any symptoms of S/I or H/I at the time of the session. QMHPA begin session by inquiring about Ms. Brown's emotional well-being. QMHPA engaged Ms. Brown in a discussion on utilizing the client-centered strategies such as active listening, empathy and validation to assist Ms. Brown with identifying her mood and discussing her triggers towards her mental illness. QMHPA assisted Ms. Brown with utilizing relaxation strategies to address her feelings. QMHPA assisted Ms. Brown with articulate her needs effectively evident by utilizing effective communication skills when feelings stressed or frustrated. QMHPA suggested to Ms. Brown that she allows herself to be changed evident by her controlling her anger before she it becomes to the point where it is uncontrollable. QMHPA engaged Ms. Brown in an activity and a discussion that would increase insight regarding how false perceptions lead to negative thoughts. Ms. Brown was open to suggestions and working on her anger for today's session. While exploring with Ms. Brown her current emotional state, Ms. Brown discussed with QMHPA she was in a positive mood. Ms. Brown stated to QMHPA that she struggles with communicating her needs at times because she feels as though she worries others with her problems and that is not what she wants to do because she knows other people have their own problems. Ms. Brown discussed with QMHPA she doesn’t like it when she is upset because she does not know how to control her mouth and when she goes off it takes her along time to come back to the person, she should be because of her mental health problems.
